The following are required sports forms for Participation with PAA. 

Sports Physicals

• PAA Pre-participation Medical Evaluation Form

Code of Conduct Attestment Forms

• Coach/Player Code of Conduct Acknowledgement Form

Concussion Awareness

• CON-02 – Concussion Information & Training for all Coaches (Required by state law)
• CON-03 – Concussion Information & Signature Form for Parents & Student-Athletes (English)
• CON-03 – Concussion Information & Signature Form for Parents & Student-Athletes (Spanish)
• CON-04 – TSSAA Concussion Return to Play Form

(This form must be completed and signed by a licensed medical doctor (M.D.), Osteopathic Physician (0.0.), or a Clinical Neuropsychologist with Concussion Training before an athlete that has been removed from practice or a game may return to participate. A copy of the form must be kept on file at the school by an administrator.)

Sudden Cardiac Arrest

• CON-02 – Sudden Cardiac Arrest  Training for all Coaches (Required by state law)
• SCA-01 – Sudden Cardiac Arrest Information & Signature Form for Parents & Student-Athletes (English & Spanish)
  
• SCA-05 – Sudden Cardiac Arrest Release Form for Return to Play 

(Before returning to practice or play in an athletic activity, the athlete must be evaluated by a Tennessee licensed medical doctor or an osteopathic physician. Clearance to full or graduated return to practice or play must be in writing.)
