Sports Forms


AS of Aug. 25, 2021, all indoor sports (Volleyball and Basketball) at any PAA sanction sports event require team members not currently on the court MUST wear a mask regardless of any local guidelines or requirements.  This includes all coaches and clock/bookkeepers.

The Parochial Athletic Association wants to ensure that, with social distancing, we can continue to reduce the spread of COVID-19.  We hope all of you, your parishes, your schools, your families, and your friends stay healthy and safe. These guidelines are meant to help school administrators and coaches make informed decisions to help prevent the spread of COVID-19 and ensure the health and safety of our athletes.

Sports Physicals
Concussion Awareness

(This form must be completed and signed by a licensed medical doctor (M.D.), Osteopathic Physician (0.0.), or a Clinical Neuropsychologist with Concussion Training before an athlete that has been removed from practice or a game may return to participate. A copy of the form must be kept on file at the school by an administrator.)

Sudden Cardiac Arrest

(Before returning to practice or play in an athletic activity, the athlete must be evaluated by a Tennessee licensed medical doctor or an osteopathic physician. Clearance to full or graduated return to practice or play must be in writing.)
